Join our compassionate team dedicated to providing exceptional care to patients in their homes. We’re seeking a skilled and empathetic Medical Social Worker to support our client in navigating the challenges of illness and medical care.

Key Responsibilities:

  • Conduct assessments to identify clients' psychological, social, and medical needs.
  • Collaborate with clients and families to develop and adjust personalized care plans.
  • Connect clients with community resources to enhance their well-being.
  • Provide support during discharge planning and case conferences.
  • Maintain accurate and timely documentation in line with agency policies.
  • Supervise social work assistants as needed and ensure the delivery of quality care.
  • Performs other duties as assigned


Qualifications:

  • Graduate of an accredited college with a Master of Social Work degree or equivalent
  • Current state licensure.
  • Minimum one year of supervised or equivalent work experience (home care experience preferred).
  • Strong communication, organizational, and problem-solving skills.
